Duplicate weapons license, certificate or card: form PPR-354

PPR-354 is the official duplicate request published by the Puerto Rico Police Bureau. Its full title is “Application for duplicate of firearm license, other licenses, certificate or card”, and that breadth is deliberate: instead of one form per document, the Police use a single one with boxes for each type. It is not a new application or a renewal; it presumes the document was already issued to you and what is being replaced is the physical copy. This guide is informational and does not describe eligibility requirements for obtaining a weapons license, which is a separate and separately regulated matter.

The form is aimed at the person who already holds the document and needs to replace it. PPR-354 identifies itself as the petitioner’s application and includes their firearm license number, with an N/A box for when the document being replaced is a different one. If you have never held the document, what applies is not a duplicate but the original application, which is a separate procedure before the Police.

• Your full names and surnames, including middle name, as they appear on the original document.
• The social security number.
• The firearm license number, or marking N/A if the document you are replacing is a different one.
• The residential address and the postal address.

   Step 1: Confirm that yours is a duplicate

   PPR-354 replaces an already-issued document. If you never held it, the applicable application is the original one — PPR-329 in the case of the weapons license — and not this form. If the document is valid but you moved, this is not it either: the Police publish PPR-1057 for change of address notification.

   Step 2: Download the form from an official source

   PPR-354 is published on two official Police sites: on the “Te podemos ayudar” page at policia.pr.gov, alongside the rest of the weapons forms, and in the document library at biblioteca.policia.pr.gov. It is the same form. Download it from there and not from a third party: a form reproduced by a blog or a seller may be out of date.

   Step 3: Mark the reason and the document

   At the top, the reason: loss, deterioration or other. Below, the document. If what you are replacing is the weapons license, write its number; if it is one of the other documents, the license number box itself has an N/A you can tick. The shooting club, gunsmith and special license boxes also carry their own N/A, so the form expects you to mark only the one that applies.

   Step 4: Confirm with the Police where to file it and what it costs

   The form states neither the cost nor the filing office, and this guide does not invent them. Confirm both with the Police before going: the switchboard is (787) 793-1234 and Headquarters is at 601 Ave. Franklin Delano Roosevelt, San Juan. The Police also operate REAL+, their electronic registry of weapons and licenses, at real-ciudadanos.policia.pr.gov.

The Puerto Rico Police administer the procedure. Form PPR-354 does not state the filing office, so confirm it with the Police before going: switchboard (787) 793-1234, Headquarters at 601 Ave. Franklin Delano Roosevelt, San Juan, 00936. The directory of state stations is on policia.pr.gov, and the electronic registry of weapons and licenses is REAL+, at real-ciudadanos.policia.pr.gov.

• Looking for a different form for the L.E.O.S.A. card, the shooting club or the gunsmith license: PPR-354 covers them all.
• Using PPR-354 for a change of address, which the Police process with PPR-1057.
• Requesting a duplicate of a document that was never issued, instead of filing the original application.
• Downloading the form from a blog or a seller instead of policia.pr.gov or the official library.
• Leaving inapplicable boxes blank instead of ticking the N/A the form provides.
• Going to file without first confirming with the Police the cost and the office, which the form does not state.
